Psychotherapy & Assessments
Fees and policies are summarized here–however, full details are available in my disclosure statement. The first office appointment is $225. On-going psychotherapy and biofeedback 45 minutes sessions are $150. Assessments are $175 per hour for administration, evaluation, and report writing.
Payment Options
Payment is expected in full at the time of service, except:
Premera, Lifewise and other Blue Cross Plans: Co-payments are due at the time of service. Any co-insurance owed will be due upon receipt of your explanation of benefits. I am a contracted, in-network provider with Blue Cross Health Plans. Coverage is 100% on most Microsoft plans.
Microsoft Cares and Wellspring Family Services EAP: Co-payments are due at the time of service. I am a preferred provider with the Microsoft Employee Assistance Program offering couple and family therapy.
For all other insurance plans: I am an out-of-network provider.
Clients may request a monthly statement listing all payments made by you and your insurance company, where applicable. This may be used to seek reimbursement, if appropriate. However, I am unable to interact with any non-contracted insurance carrier in any way.
Most major credit cards, debit cards, flexible spending accounts and health savings accounts are accepted forms of payment for services.
Cancelations
Please note: Office appointments are reserved spaces. Cancelations must be made 48-hours prior to your appointment to avoid a charge for the session.
